Chris Hildebrandt Professional Corporation (“we,” “us,” or “our”), operating as Chris the Chiro, provides professional chiropractic services at Altitude Collaborative Health and Action Health. We are committed to protecting the privacy and security of our patients’ personal health information (PHI) in accordance with the Alberta Health Information Act (HIA) and the Personal Information Protection and Electronic Documents Act (PIPEDA).

1. Accountability
We are responsible for all personal health information under our control. Our designated Privacy Officer is Chris Hildebrandt, who oversees compliance for all practice locations.

2. Information We Collect
We collect health information necessary to provide care, including identification, health history, and assessment records. This data is managed through two secure, PIPEDA/HIA-compliant electronic health record (EHR) systems:
Action Health: Data is managed via Atlas EHR.
Altitude Collaborative Health: Data is managed via Jane EHR.

3. Email Communications (CASL Compliance)
In accordance with Canada’s Anti-Spam Legislation (CASL), we only send commercial electronic messages (emails) to patients who have provided express consent.
Purpose: We use email for appointment confirmations, sending exercise programs, sharing clinic newsletters, and providing health-check reminders.
Security & Risk: While we use secure systems, standard email is not a 100% secure method of communication. By consenting to email, you acknowledge the risk of interception. We avoid sending sensitive diagnostic results via unencrypted email.
Unsubscribe: Every marketing or newsletter email includes a clear “Unsubscribe” link. We honor all opt-out requests within 10 business days.

4. SMS Messaging & Consent (CASL Compliance)
In accordance with Canada’s Anti-Spam Legislation (CASL), we only send text messages to patients who have provided express consent.
Consent Method: Consent is obtained explicitly through the secure intake and profile settings of either Jane EHR or Atlas EHR, depending on the clinic location.
Opt-Out: You may withdraw consent at any time by replying STOP to any text.
Purpose: SMS is used for care coordination, scheduling assistance, and health-check reminders. We do not send marketing “blasts” to non-patients.

5. NO SHARING OF MOBILE DATA
Chris Hildebrandt Professional Corporation does not sell, rent, or share your mobile information or SMS opt-in consent data with third parties or affiliates for marketing or promotional purposes. All patient data is used strictly for internal care coordination between the patient and Chris Hildebrandt Professional Corporation.

6. Data Storage & Security
All health information is encrypted at rest and in transit. Both Jane and Atlas EHR systems utilize secure Canadian-based servers. We maintain strict administrative, technical, and physical safeguards to prevent unauthorized access.
